site. First come first served.
FOR THE “GOLD CARD” YOU MUST BRING*:
1.

One valid
Texas ID for you and for your spouse, if applicable
2.
Birth certificates for dependents
3.
TWO current documents/ bills showing where
you live for
the last 60 days
4.
ALL documents showing income and/or paycheck
stubs for
the last 30 days
5.
ALL insurance documents, if applicable
6.
ALL INS (Immigration) documents, if
applicable
7.
Medicaid Letter for current month, if
applicable
8.
Medicare Identification Card, if applicable
9.
Social Security Award Letter
10. Bank
statements (dated for the last 30 days), if applicable
11. ALL credit
card statements (for the last 30 days), if applicable
12. Food stamp
certification or TANF Certification, if applicable
