About SBCHCSpring Branch Community Health Center is a private, non=profit community health center providing quality and affordable healthcare services to the underserved community of Spring Branch. Our mission is to provide comprehensive healthcare services to all patients and to become the healthcare home for residents in this community. History of SBCHCIn 2003 a group of community leaders decided to take action to help reduce healthcare disparities by creating a neighborhood base health center. With the financial support of neighborhood churches and private donations the clinic opened its doors in May 2004 offering services three days a week. In 2008 the health center federal grant was renewed for an additional three years. Service Delivery ModelSBCHC offers an integrated and comprehensive service delivery system for the provision of health care, mental health, dental referrals, and substance abuse services that is client driven, culturally sensitive and family focused. SBCHC works to improve the health conditions affecting the target population primarily obesity, high blood pressure, diabetes, and depression. The model enables SBCHC to provide the most cost effective, preventive and caring services to those in the community with the greatest needs, while at the same time, ensure the continuity of care and coordination between services. New Patient requirementsNew patients need to complete a patient registration form. We accept most private insurances, Medicaid, and Medicare. Appointments are recommended but we do accept walk ins. SBCHC offers a sliding fee discount for patients who meet the requirements. Patients will need to show proof of income to qualify. Requirements include a photo ID, check stubs for the latest month of every individual that works in the household and proof of residency.
